Student Athlete Questionnaire
The Gordon Athletics Department seeks each year to more effectively communicate and disseminate information regarding all things Gordon Athletics to various media outlets. Having information about each of our student athletes on file is essential to our success in that endeavor. The following form will help to that end and should be completed by each student athlete prior to the first competition date of his/her respective season:
